DISABILITY STUDIES CERTIFICATE Application Form

The Disability Studies Certificate application form must have the appropriate signatures prior to submission to the IHDD. Refer to the Disability Studies Student Handbook for additional information.

1. Complete the application,
2. Print the form,
3. Secure applicable signatures, and
4. Send to Dr. Manders, IHDD, for approval.
   
   (Form with original signatures required.)
